Abnormal bleeding is a common problem among women at some time during life. The causes of abnormal bleeding vary by age, with different common causes in young girls before their first period, versus younger reproductive age women, versus older reproductive age women, versus post-menopausal women. All bleeding in young girls before their first period and all bleeding in post-menopausal women is abnormal and should be evaluated. Among reproductive age women, bleeding that is heavier than usual, longer than usual, and/or more frequent than usual for each woman is also abnormal, with most women expected to have a moderate amount of menstrual bleed lasting < 8 days about once per month, occuring with predictable timing. In addition to potential problems with the heaviness, length or frequency of bleeding, some women skip periods, going 2-3 months or longer between bleeding episodes; this is also abnormal bleeding, needs evaluation and is commonly assocaited with decreased fertility, among other medical problems.
There are many causes of abnormal bleeding. Evaluation of each patient is individualized based on each woman's symptoms, age, menopause status, medical history, and findings during evaluation. Some common causes of abnormal bleeding include: fibroids, endometrial or cervical polyps, adenomyosis, neoplasia/cancers, abnormal ovulation, bleeding disorders, and medications.
